The Nurse’s Role in Ibogaine Treatment: 24/7 Care, Safety, and Compassion
While the attending physician sets the clinical protocol and the therapist guides the psychological journey, the nursing staff forms the absolute backbone of a safe ibogaine treatment experience. They are the constant, unwavering presence by the patient’s side, tasked with mitigating safety risks 24 hours a day, 7 days a week.
In our facility, nurses do far more than just check vitals. By combining rigorous medical monitoring with deep, specialized psychological training, our nurses create an environment where patients feel physically protected and emotionally supported.
Here is an inside look at the vital role our nursing team plays, featuring insights from Karen Jackalina Ortiz Leon, a 25-year-old general nurse at our clinic.
1. Specialized Cross-Training: Medical Meets Psychological Care
Ibogaine therapy is unique because the intense physical effects are deeply intertwined with complex psychological breakthroughs. To manage this safely, a standard nursing background is not enough.
At our clinic, all nurses undergo specialized training led by our lead therapists and expert ibogaine treatment guides. This interdisciplinary training ensures that our nursing staff fundamentally understands the root conditions our patients are battling.
Whether a patient is seeking treatment for severe drug addiction, deep-rooted trauma and PTSD, chronic depression, or various personality disorders, our nurses are trained to recognize the behavioral and emotional nuances of these conditions. This allows them to provide empathetic, highly tailored care that aligns perfectly with the patient’s overarching therapeutic goals.
2. Pre-Treatment: Building Trust and Managing Anxiety
The morning of the treatment is often the most nerve-wracking time for a patient. Long before the medicine is administered, the nurse’s work begins.
As Karen explains, establishing a bond of trust is the first clinical objective of the day:
“My support during the ibogaine process starts beforehand. It starts in the morning by building that bond of trust with the patient, answering all the necessary questions about what they can expect… It is very normal for the patient to be very anxious all morning and right up until minutes before starting the treatment.”
Because anxiety can trigger physical responses (like an elevated heart rate), the nurses use advanced telemetry monitors to track both physical safety and emotional distress.
“When we actually attach the monitor, we’ll see the normal data. We need to check their heart rate to ensure it’s within the normal range, as well as their breathing and oxygen saturation, which we are constantly monitoring for any issues or abnormalities. If the patient is very anxious, the monitor can also indicate that.”
3. Mitigating Safety 24 Hours a Day, 7 Days a Week
Once the treatment begins, the patient enters a highly vulnerable, oneirogenic (dream-like) state. During this time, they are never left alone.
Our facility enforces a strict protocol where nurses mitigate safety 24 hours a day, 7 days a week. They maintain a constant, vigilant watch over the telemetry screens, ensuring the patient’s heart rate, blood pressure, and oxygen levels remain perfectly stable while the doctor’s prescribed protocol is carried out. If a patient needs to move or use the restroom, a nurse is physically there to support them, ensuring zero risk of falls or injury during the physically disorienting phases of the medicine.
4. Post-Treatment Care: Navigating the “Hardest Day”
The active phase of ibogaine is intense, but the true physical and emotional challenge often arrives the following day. As the medicine continues to process in the body, the patient is left facing their raw, unfiltered self.
“The next day is usually the hardest, to be honest. I mean, I can’t guarantee patients will feel refreshed and 100% happy because the symptoms might not be that pleasant… they’re getting to know what the person is really like, how you’re feeling with your true self.”
During this critical window, our nurses shift into intensive recovery mode. Common physical symptoms during the “gray day” (the day after treatment) can include:
-
Nausea and excessive vomiting
-
Physical tremors
-
Headaches
-
Loss of appetite
-
Dehydration
To combat this, the nursing staff remains in constant communication with the patient, providing continuous post-treatment monitoring and administering IV hydration and anti-nausea medications to keep them as physically comfortable as possible while their brain recalibrates.
5. The Transformation: A Clinic That Feels Like Home
Despite the intense medical requirements, a sterile hospital environment is not conducive to deep psychological healing. Our nurses work tirelessly to ensure the facility feels safe, warm, and inviting.
“More than a clinic or a hospital, it’s a home. So, we provide the best care possible as if they were our own family so they can come and get to know themselves… It is very rewarding to witness the change in these individuals. Honestly, when you can really see the change both physically and mentally. I could say they’re like different people.”
For the nursing staff, the ultimate reward is seeing a patient who arrived burdened by severe trauma or addiction leave the facility entirely renewed. As Karen notes, “We often tell them, ‘Wow, you’re a new person, right? You’re leaving different.’ Yes, the change is definitely noticeable.”
